Speaker bio: D’Leah Cruz BSN, PMHRN-BC, QMHP


D’Leah Cruz, is a Clinical Program Manager RN with Providence Home Health. She oversees the Mental Health Nurse Program and Certified Wound Ostomy Nurse Team. She has been working in enhancing safety in healthcare setting since 1991.

 

She came to Providence Health & Services in 2014 as the manager for the Chemical Dependence Program and joined Providence Home Health in 2019. She was formerly with the State of Oregon where she was part of the state hospital admissions and community placement program. D’Leah co-developed a model for crisis intervention teams to help people move from the state psychiatric hospital to community care. 

 

D’Leah has experience in secure residential treatment and inpatient acute care mental health hospitals, residential care, assisted living, adult foster homes, direct care nursing and clinics. She is passionate about making the workplace safer for healthcare workers who assist families and people with health issues, trauma, or lack of behavioral and medical resources.

We plan for many things in life, retirement, accidents, known physical health risk and financial security for our families and loved ones. With long life we have seen an increase in cognitive issues, specifically Dementia. Are you informed and prepared, do others know what you would want?
